The Consumer Account Request must include the following:

  • Company's name and complete address
  • Requestor's name and telephone number
  • First Federal customer's name and address (for whom the information is requested)
  • First Federal account number and customer’s Social Security Number or Federal Tax ID number
  • Borrower's authorization should be in the same name as the entity requesting the verification, or provide the listing of relationship of authorized parties to request information.
  • First Federal customer's signature (s), we cannot process requests without proper authorization by our customer.
