When submitting your request, please include the following:

  • Requesting company's name and complete mailing address,
  • Requestor's name and telephone number,
  • First Federal customer's name and address (for whom the information is requested),
  • First Federal account number and customer’s Social Security Number or Federal Tax ID number,
  • Borrower's authorization must list the company requesting the verification, or provide listing of relationship of authorized parties to request information,
  • First Federal customer's signature (s), we cannot process requests without proper authorization by our customer to submit your completed request form.
